A 46, XY disorder of sex development (DSD) is a condition in which an individual with one X chromosome and one Y chromosome in each cell, the pattern normally found in males, have genitalia that is not clearly male or female.

Karyotype Test
Also called: Chromosome Analysis, Chromosome Karyotype, Chromosome Studies, Chromosome Testing, Karyotyping, Karyotype Genetic Test
A karyotype test is a type of genetic testing. It looks at the size, shape, and number of your chromosomes. Chromosomes are parts of cells that contain genes. If the number or structure of your chromosomes is not normal, it can indicate a genetic disease.

Luteinizing Hormone (LH) Blood Test
Also called: LH Test, Luteinizing Hormone, Lutropin, Interstitial Cell Stimulating Hormone, ICSH
This test measures the level of luteinizing hormone (LH) in the blood. LH plays an important role in sexual development and functioning. Levels that are too high or too low can cause health problems for men and women, and delayed or early puberty in children.
Testosterone
Also called: Testosterone Levels, Serum Testosterone, Testosterone, Total
A testosterone levels test measures the amount of the hormone testosterone in a sample of your blood. Testosterone is the main sex hormone in men. Women also have testosterone in their bodies, but in much smaller amounts. Low or high levels may be a sign of a health problem.

Anti-MĂĽllerian Hormone (AMH) Test
Also called: AMH, AMH Hormone Test, Anti-MĂĽllerian Hormone, MĂĽllerian-Inhibiting Hormone, MIH, MĂĽllerian Inhibiting Factor, MIF, MĂĽllerian-Inhibiting Substance, MIS
An anti-mĂĽllerian hormone (AMH) test measures the level of AMH in the blood. It is often used to check a woman's ovarian function and fertility. AMH also plays a key role in the development of sex organs in an unborn baby.
Dehydroepiandrosterone Test
Also called: DHEA, Unconjugated DHEA
The test measures the levels of dehydroepiandrosterone (DHEA) in blood. DHEA is an adrenal hormone that makes androgens and estrogens, the male and female sex hormones. Under normal conditions, the levels of DHEA decreases as the person ages. Increased levels can be associated with certain health problems.
Inhibin B Test
Inhibin is a hormone produced by the ovaries and testes. There are two types of inhibin, which are termed inhibin A and inhibin B. Abnormal levels of the inhibin B hormone can be related to infertility, ovarian cancer, low production of sperm, and several other conditions.

The 46, XY disorders are a group of different conditions including 46, XY complete gonadal dysgenesis (Swyer syndrome), 46 XY, partial gonadal dysgenesis (Denys-Drash syndrome, Frasier syndrome), ovotesticular DSD, testicular regression syndrome (vanishing testes syndrome) , Leydig cell aplaisa/hypoplasia, testosterone biosynthesis defects, POR gene abnormality, persistent Mullerian duct syndrome, 5 alpha-reductase type 2 deficiency and complete and partial androgen insensitivity syndromes.
Therefore, several biologic factors such as genetic and hormonal influences contribute to gender identity, gender role behavior, and sexual orientation in humans, but this relationship is also modified by psychological, social, and cultural factors. Sex assignment of children with ambiguous genitalia remains a difficult decision for the families involved and subject to controversial discussion among professionals and self-help groups. Every case is unique and it is suggested that gender assignment should be based on clinical experience, study of the literature, and interviews with affected individuals.
In a large study recently published about women with complete androgen insensitivity syndrome (CAIS) it was found that they perceive themselves as highly feminine throughout development. Additionally, CAIS women largely report their sexual attraction, fantasies, and experiences were best described as female heterosexual. CAIS women unanimously reported satisfaction with being a woman (100%). Another study with 46, XY individuals with androgen secreting testicular tissue and 46,XX individuals with congenital adrenal hyperplasia concluded that androgen exposure may have some effect in higher brain function and the perception of the gender. The authors suggested that gender counseling should be part of the routine management for these patients to make sure that they are happy with being females.
